To apply for membership, print out this form, fill in your details, (if your printer is not working, just send a letter) and send it by mail to the Hon. Sec. enclosing your subscription. The current Annual Subscription, payable at the beginning of each calendar year, is 15 Euros.

If it is difficult for you to send 15 Euros, you may send a cheque or cash for £10 Sterling or US$20 or AUS $25 or CAN $30 or NZ$ 25.

Anyone who is connected with
or interested in
a family of the name Butler
is entitled to become a member.
